Registering your child's (under 16) change of name online

Before you begin your online application, please read the following carefully.

Important Information

  • If your child is under 6 months old, please contact BDM for advice about correcting the original birth registration.
  • Once you start the application, you cannot save and return to it later.
  • If you're inactive for 30 minutes, your session will expire, and you'll need to start again.
  • If your payment fails, you must restart the application.

What You'll Need

Have the following documents and details ready before you begin:

Proof of Identity

  • A clear, readable scan or photo of the applicants ID. Some identification require both front and back.  You can verify your ID online using the Digital Verification Service or you can upload it manually.

Evidence of Previous Names

  • Documents showing any previously registered names your child has used.

Payment Details

  • Credit card information including:
    • Cardholder name
    • Card number
    • Expiry date
    • CVV number

Eligibility

  • You must not have registered a change of name for your child in the past 12 months.

Consent

  • If the child is 12 or older, they must fill out the consent form for changing their name.  They need to sign it in front of a Justice of the Peace or a Commissioner for Declarations.

Name restrictions

The Registrar may refuse your application if the proposed name is considered a prohibited name.

Application fees are non-refundable, except as outlined in our Refund Policy.
