Report a concern about child safety

Before you start the form

  • Use this form to report child safety concerns relating to Department of Justice employees, volunteers and staff operating under a Service Level Agreement (or similar contractual arrangement) with the Department of Justice.
  • Anyone, including Department of Justice staff and members of the public who may have witnessed concerning or inappropriate behaviour, can use this form.
  • The form has 10 questions. The progress bar at the top of the screen will show you how far through you are. Questions you need to answer to be able to submit the form, are marked with an asterisk (*).
  • Provide as much detail as you are comfortable with.
  • If you want to, you can fill out the form without telling us your details. If you do provide your details, it may help the Department of Justice to better respond to the concern.
  • The form will not be able to be saved and returned to later.
  • When you submit the form, a summary will be sent to secretary@justice.tas.gov.au.

What the form will ask you

You’ll be asked to tell the Department of Justice:

  • Who the report is about, including the person’s role within the Department of Justice, or what area of the Department the person works.
  • As much detail about the person’s conduct as you can, and feel comfortable providing (what the person did, who it was directed towards, if they were with anyone else).
  • How you would like to be contacted (if you would like to be contacted) if the Department has more questions about what happened.
  • Who else (if anyone) you’ve talked to about your concern (if you’ve told police or have called the Strong Kids, Safe Families Advice and Referral Line).

What happens after you fill in the form

  • If you fill in the form to report a concern about child safety and wellbeing related to the Department, you’ve done the right thing.
  • You’ll get a confirmation email after you’ve submitted the form (if you’ve provided your contact details and indicated you want to receive a confirmation email). If need be, we might contact you to ask for further details.

Personal information collection statement

You are providing personal information to the Department of Justice (such as your name, email address and/or phone number).

We collect and use that information in accordance with the Personal Information Protection Act 2004 and the Department of Justice’s Personal Information Protection Policy.

Your information may be shared with other relevant persons such as the Independent Regulator and Tasmania Police, in accordance with the Child and Youth Safe Organisations Act 2023.

We take reasonable steps to ensure that the personal information we hold is accurate, complete and up to date. Where practicable, we will check on the accuracy of personal information before we use it.
