The Freedom of Information Act 1991 (FOI Act) gives you the right to:
obtain access to information held by this Department (and by other Tasmanian agencies including government departments, ministers, local councils and other public authorities);
request amendment to personal information held by those bodies that is inaccurate, incomplete, misleading or out of date;
seek a review by the body or by the Ombudsman of a decision where access to information has not been granted or a request to amend personal records has been refused.
Applications for access to or amendment of information need to be made to the relevant department, authority or council.

Requests to access information held by other Tasmanian agencies should be directed to the FOI Officer in the Agency to which you are applying.
While it is not essential to use a particular form, a FOI Request Form can be downloaded from the link below. The same range of information could be included in a letter requesting the information but the use of the request form will assist in formulating your request and also assist the agency in resonding to the request.
