

Application for Election for Court Hearing form
If you wish to have the offences on an infringement notice determined by a court, or you wish to argue that the penalty should be reduced, you must make your application using this form.
This is a formal application to the court and will require your attendance in court when it is heard.
You cannot elect a court hearing if you have already paid the penalty in full or part or applied for a variation of payment conditions.
Application for Variation of Payment Conditions form
If you can’t pay an infringement notice or court fine within the time allowed, you should apply to pay by instalments. Unless you are requesting assessment for an MPCSO, initially do this by phone. You need only complete this Application for Variation of Payment Conditions form if you are requesting assessment for an MPCSO or if asked to by MPES.
Application For Suspension of an Enforcement Order form
If you believe that an enforcement order should be suspended, you can apply for it to be suspended using this form. If suspension is granted, you have 28 days to take further action to deal with the penalty.
Authority for an Agent to make enquiries on your behalf
If you would like someone else other than you to discuss your monetary penalties with MPES, you should complete this form and return it to the Monetary Penalties Enforcement Service.
