Commission of Inquiry : Relating to the Inquest into the Death of Joseph Gilewicz

The Commission of Inquiry was established pursuant to the provisions of the Commissions of Inquiry Act 1995 with terms of reference requiring it to inquire into:
- Whether, in the light of material presently available but not received by the coronial inquiry, the coronial inquiry into the death of Joseph Gilewicz was likely to be misled by the evidence tendered to it;
- If the coronial inquiry was likely to have been misled, to what extent is that likely to have impacted upon the finding of the coroner.
The Hon. Dennis Mahoney AO QC was appointed the sole commissioner.
