For adult (18 years and over)
The applicant must provide three personal identity documents. At least one document must be from List 1. The other two documents may be from either List 1 or List 2.
If all identity is in the new name, the applicant must establish proof of their original identity (i.e. additional ID in the old name). If the applicant is born outside Tasmania they must provide evidence of at least 3 months residence in Tasmania.

Any person born outside Tasmania must supply a Birth Certificate.
Original documents must be supplied if application is lodged in person at the Registry Office. If application is lodged by post, photocopies MUST be certified as a "true and correct copy" by a Justice of the Peace or Registry Office Staff.
For child (under 18 years)
A Change of Name of a child between 12 & 18 years of age requires written consent of the child in a form approved by the Registrar of Births, Deaths and Marriages and it must accompany the application form.
Each parent or legal guardian applying on behalf of a child must establish their OWN identity and provide one document from List 1.

Parents applying for children 5 years and over must establish the identity of each child by providing two documents from List 2. For children under 5 years one document must be produced from List 2. The child's documents may be in the old or new name.
