Identification requirements for Tasmanian Birth, Death and Marriage Certificates
Acceptable types of Australian identification are as follows. Applicants must provide either one document from List 1 or two documents from List 2. If application is received by mail or fax then photocopies only are to be provided.
LIST 1:
One of the following:
- Current Australian photo driver's licence
- Current Australian passport
- Current Document of Identity issued by the Passport Office
- Current Defence Force or Police Service photo ID card
LIST 2:
Two of the following:
- Current Medicare card
- Current credit card, account card, bank passbook or account statement
- Current entitlement card issued by the Commonwealth Government
- Current student identity card or statement of enrolment
- Tasmanian proof of age card
- School or other educational report, less than twelve months old
- Current overseas passport, with current entry permit
- Naturalisation/citizenship/immigration papers issued by the Department of Immigration
- Council land rate or water rate notices, electricity or other utility account
