Identification requirements for Tasmanian Birth, Death and Marriage Certificates

Acceptable types of Australian identification are as follows.  Applicants must provide either one document from List 1 or two documents from List 2.  If application is received by mail or fax then photocopies only are to be provided.

LIST 1:
One of the following:

• Current Australian photo driver's licence
• Current Australian passport
• Current Document of Identity issued by the Passport Office
• Current Defence Force or Police Service photo ID card

LIST 2:
Two of the following:

• Current Medicare card
• Current credit card, account card, bank passbook or account statement
• Current entitlement card issued by the Commonwealth Government
• Current student identity card or statement of enrolment
• Tasmanian proof of age card
• School or other educational report, less than twelve months old
• Current overseas passport, with current entry permit
• Naturalisation/citizenship/immigration papers issued by the Department of Immigration
• Council land rate or water rate notices, electricity or other utility account
