Adding biological father's details to a Birth Certificate

It is possible to add the biological father's details to a child's Birth Certificate.  A statutory declaration 'Adding Biological Father's Details to a Birth Record' can be obtained by contacting the Registry.

To apply you must lodge the following documentation with the Registry of Births, Deaths & Marriages:

  • a completed statutory declaration to 'Add Biological Father's Details to a Birth Record'.  Both parents must sign the form in the presence of Justice of the Peace or Commissioner for Declarations
  • a certified copy of the child's birth certificate
  • identification for both parents, eg.
      Passport
      Driver's Licence
      Tax Certificate
      Citizenship Certificate
      Defence Forces or Police photo ID
      Social Security Benefits card
  • the prescribed fee

Please Note:  Changing a child's name or adding marriage details to the Birth Certificate is a separate process.
